Change of Address Form
Instructions: If you have a permanent change of address and need to inform the school, please use the attached form to make the necessary changes. Fill in all sections legibly (click the 08 in the date below to see the form). Remember, bus service is tied to the home address that is entered into your child’s records. Students must be picked up and dropped off at the stop closest to your home.
Parents may drop the form off to the school’s main office or you may send it back to school with your child. Your child’s homeroom teacher will see that it is presented to the office staff for updating.
NO Change of Address Form can be submitted over the Internet.
