Parents or guardians wishing to enroll their child(ren) for the first time at Fannie C. Williams Charter School will need the following documents to complete the process:
• One-App (RSD Application)
• Copy of the student’s birth certificate
• Copy of student’s Social Security Card
• Copy of student’s immunization record
• Copy of three proofs of address (e.g., utility bill, telephone bill, cable bill, etc.)
• Copy of final report card
• Copy of current Individualized Education Plan (IEP)–if applicable
• Copy of most recent IOWA, iLEAP, and LEAP test scores
• Copy of current Individualized Accommodation Plan (IAP/504 Plan)–if applicable
